Based on your needs, you will receive a recommendation for the appropriate level of support.

Begin Your Planning or Support Process
Terry will walk you through the agreed-upon planning, organization, or family-support process.

Review Your Information
You remain in control of your decisions. Review your wishes, information, and next steps before anything is considered complete.

Communicate and Keep Information Accessible
When appropriate, communicate important information with your family or other trusted individuals and store your information somewhere accessible.

Simply Prepared supports clients in organizing information and communicating their wishes. Clients remain responsible for their own decisions.
A family member cannot sign documents, provide consent, authorize services, or make decisions for another competent adult unless that person has valid legal authority to do so.
When someone has a legally authorized representative, appropriate documentation may be required before that representative can act on the client's behalf.
Simply Prepared does not determine whether someone has legal capacity and does not create powers of attorney or other legal authority documents.
